Emergency Mental Health Shelter Care
Crossroad is licensed to place children, ages 6-21, in our Emergency Mental Health Shelter Care. Referrals must be made by the Indiana Department of Child Services or Juvenile Probation.
The Emergency Mental Health Shelter is considered a short-term placement only for children in active crisis situations. While in our care, children will receive services to meet basic needs for food, safety, and shelter, among others.
Placement is available for 20 days unless otherwise approved by the DCS Deputy Director of Services. Children will be placed in the Emergency Mental Health Shelter based on availability.
An intake packet with information about the child must be completed and submitted to Crossroad. If available, these items should also be submitted:
- CANS Assessment
- Case Plan
- Medical Passport
- IEP
- Child Summary
